If you wish to be registered as a patient, the process is simple. Give your signed medical card to the receptionist and you are registered. You will be invited to see the practice nurse for a “New Patient” assessment. Although you do not need to accept this offer, most new patients find it very beneficial. It allows you to meet us and allows us to assess your medical needs.

If you have lost your medical card, just ask at reception for a form to get a replacement card. You can still receive treatment.
